Sildenafil and tadalafil, the active ingredients in Viagra and Cialis, remain the most reliable treatments for erectile dysfunction. Sanofi — the pharmaceutical giant that makes Cialis — is currently designing a study that would put them on track to alter Cialis’ designation from a prescription drug to over-the-counter medication.
Ginkgo biloba. Known primarily as an herbal treatment for cognitive decline, ginkgo has also been used to treat erectile dysfunction -- especially cases caused by the use of certain antidepressant medications. But the evidence isn't very convincing. One 1998 study published in the Journal of Sex & Marital Therapy found that it did work. But a more rigorous study, published in Human Pharmacology in 2002, failed to replicate this finding. "Ginkgo has come out of fashion in the past few years," says Ronald Tamler, MD, assistant professor of medicine and codirector of the men's health program at Mount Sinai Medical Center in New York City. "That's because it doesn't do much. I can say that in my practice, I have not seen ginkgo work -- ever."

Among the most popular treatment options for erectile dysfunction are pharmaceuticals. Drugs such as Viagra (Sildenafil), Levitra (Vardenafil), and Cialis (Tadalafil) are popular and boast an 80% success rate. These drugs are classified as phosphodiesterase-5 inhibitors (PDE-5) – which enable an erection, temporarily, but are not the best for maintaining an erection over time.
ED treatments block the action of PDE5, allowing more blood to flow into the penis. The resulting erection is firmer and usually lasts long enough to allow for satisfying sex.
Even if you take the medicine, you still need physical and mental stimulation and desire to have an erection. If your first dose doesn’t help, call your doctor. Your doctor may want to change your tablet strength.
Then the men were given both L-arginine and pycnogenol for a month. After this second month, 80% of the men reported that their sexual function was fully restored. Using this same combination for a third month, over 92% of the men had what they considered normal erections.
The FDA warns that some products may contain harmful substances or the active ingredient in some prescription medications.

Testosterone. It’s a male hormone. If you have a normal testosterone level, you don’t need more.
Many factors can contribute to sexual dysfunction in older men, including physical and psychological conditions, comorbidities and the medications used to treat them. Aspects of an ageing man’s lifestyle and behaviour and androgen deficiency, most often decreasing testosterone levels, may affect sexual function as well. A study of men between the ages of 30 and 79 years showed that 24% had testosterone levels below 300 ng/dL and 5.6% had symptomatic androgen deficiency.2
