The first stem cell study for the treatment of ED was published in 2004. This study used embryonic stem cells to treat ED. At this time, there is a total of 36 published basic studies assessing stem cell therapy for ED, with two clinical trials. The mechanism of action of stem cells is to generate angiogenesis with subsequent increase in cavernosal smooth muscle cells within the corporal bodies.46

The inability to achieve an erection is a symptom of an underlying health problem or a result of lifestyle factors that aren’t allowing the physiological mechanisms of an erection to unfold. For some men, this is only an occasional problem, which may be the result of increased stress, drinking alcohol or fatigue. But for men who experience erectile dysfunction more than 50 percent of the time when they are trying to engage in sexual activity, this may indicate that there’s a bigger health problem that needs to be addressed.

These causes of ED can be categorized into two types: psychogenic (mental) and organic (physical). Psychogenic causes include depression, low self-esteem, anxiety, and negative feelings about sex.Organic causes account for about 80 percent of ED cases and can be further classified as endocrine or non-endocrine. Endocrine-related causes are often connected to low testosterone levels, though more research is required to fully understand how the two intersect. Of the non-endocrine related causes of ED, vasculogenic causes, which affect blood supply and flow, are the most common. How to Get Diagnosed With Erectile Dysfunction
A study published in the January 2016 edition of the American Journal of Clinical Nutrition suggests a diet rich in flavonoid-rich foods can reduce the risk of ED. The study reviewed data from more 25,000 men who were middle-aged and older, and who participated in routine health survey since 1986. Researchers compared responses about having and maintaining an erection sufficient for intercourse with the number of flavonoid-rich foods each man reported consuming. Men who ate more fruit had a 14 percent reduced risk of ED. Men who consumed a high volume of flavonoid-rich fruits and exercised regularly experienced a 21 percent reduction of risk. Foods such as blueberries, cherries, blackberries, radishes, citrus fruits, and red wine offered the flavonoid profile with the greatest benefits. L-arginine is an amino acid that naturally occurs in the body and helps synthesize proteins. It is also the precursor to the compound nitric oxide (NO). NO is vital for achieving and maintaining an erection, as it helps the blood vessels relax to support blood flow throughout the body, including into the penis. Supplementing arginine in conjunction with a balanced lifestyle can offer men added support for a healthy erection. Added pounds can have a significant impact on blood flow, as well as put men at risk for diseases that may exacerbate ED. A study published in the January 2005 issue of the journal Canadian Family Physician found 30 percent of men who participated in a weight loss program and lost an average of 15 percent of their total body weight saw sexual function restored compared to only 5 percent of the control group.

Management of EDOptimal management of ED involves shared decision-making between the patient, his partner, and the treating clinician.6 Underlying disease states and risk factors should be identified and treated or minimized.5 Contributing medications should be discontinued, or the dose should be reduced.5 A variety of nonpharmacologic and pharmacologic treatment options may be considered for ED treatment. Generally, oral medications are trialed first, and more invasive options are reserved for those who fail to respond to initial therapy.5,6
Among the most popular treatment options for erectile dysfunction are pharmaceuticals. Drugs such as Viagra (Sildenafil), Levitra (Vardenafil), and Cialis (Tadalafil) are popular and boast an 80% success rate. These drugs are classified as phosphodiesterase-5 inhibitors (PDE-5) – which enable an erection, temporarily, but are not the best for maintaining an erection over time.
In the past, erectile dysfunction was considered to be a psychological issue about 90 percent of the time. But as physicians learned more about erectile dysfunction — and as effective treatments were developed — today, only about 10 percent of cases are considered to have psychological causes.
Bullet point: The relationship between the risk factors for CVD and ED are intimately interwoven, one with the other. Clinicians need to factor this in when counselling the man with ED.
