Often known by the common brand name Cialis, tadalafil is a PDE-5 inhibitor that you take once per day to provide 18-36 hours of effectiveness. This significantly reduces the planning typically involved in using other treatments like sildenafil. That said, some users only take tadalafil as needed, and those dosages can sometimes be different. You and your doctor will decide what’s right for you.

Erectile dysfunction (ED, or impotence) is a condition that causes men to be unable to acquire or maintain an erection.
Penile implant surgery can be very effective, provided that precautions are taken to avoid infection. Prosthesis surgery is contraindicated if systemic cutaneous or urinary infection is present. Antibiotics should be provided pre-operatively, and the surgical site should be shaved immediately prior to surgery. We use both Mentor and AMS penile implants with specialized antibiotic coats. Patients are hospitalized overnight for closed suction drainage, and sent home on seven days of post-operative antibiotics. Using these and other precautions, our implant infection rate is comparable to national averages (2-4%, 1-2% for antibiotic coated implants). Vascular surgery is recommended only in healthy individuals with recently acquired erectile dysfunction due to a focal arterial narrowing (usually related to trauma) and in the absence of generalized vascular disease. Erectile dysfunction medications are effective for many men, but these oral medications may not work for everybody. Our team offers numerous other non-invasive and minimally invasive treatments for improved outcomes. Many of these can achieve results faster, with fewer side effects and a quicker recovery. We also offer placement of inflatable penile prostheses for men who have failed other forms of treatment for erectile dysfunction.
The next new treatments for erectile dysfunction will probably be improvements in some ED drugs already being used. "A dissolvable form of Levitra that you put under your tongue is coming that may work more quickly than the pills we have now," says Feloney. A new form of alprostadil may make it possible for you to rub it directly on the penis instead of inserting or injecting it. And newer phosphodiesterase inhibitors that last even longer and cause fewer side effects are being developed. Stay tuned!
Viagra is usually taken only when needed, 30 minutes to 1 hour before sexual activity. You may take it up to 4 hours before sexual activity. Do not take Viagra more than once per day.
Preference for Viagra isn’t just a sugar-pill thing, either; there are differences in dosage to consider. Viagra commonly comes in 25mg, 50mg, and 100mg doses, with a typical dose of 50mg prescribed at the outset for men under 65. Sildenafil, meanwhile, comes in a broader range of options, including 20, 30, 35, 60, 80, and 100 mg. So, it’s the same active ingredient (sildenafil), which acts in the same way in the male body. But you may experience different levels of efficacy and side effects depending on the dosage.
The pathogenesis of organic ED is related to dysfunction of the endothelium. Endothelial cells can become injured through a variety of mechanisms, most of which cause oxidative stress on the tissues. Many of these causes of oxidative stress are related to lifestyle issues which lead to hypertension, diabetes and dyslipidaemia (figure 1). Endothelial cell dysfunction results in reduction of endothelium-dependent vasorelaxation as well as increased adhesion of leukocytes to the endothelium. Endothelial cell injury then leads to a variety of sequelae, including ED, other types of vasoconstriction, atherosclerosis and thrombus formation.18
