The pathogenesis of organic ED is related to dysfunction of the endothelium. Endothelial cells can become injured through a variety of mechanisms, most of which cause oxidative stress on the tissues. Many of these causes of oxidative stress are related to lifestyle issues which lead to hypertension, diabetes and dyslipidaemia (figure 1). Endothelial cell dysfunction results in reduction of endothelium-dependent vasorelaxation as well as increased adhesion of leukocytes to the endothelium. Endothelial cell injury then leads to a variety of sequelae, including ED, other types of vasoconstriction, atherosclerosis and thrombus formation.18

Tests for underlying conditions can include: A physical exam: Your doctor will examine your penis and testicles for signs of abnormalities and will test sensation. Blood tests: Blood tests can show if you may have heart disease, diabetes, low testosterone levels, or other issues. Urine tests: Urinalysis is used to look for signs of diabetes and other health conditions. An ultrasound: A specially trained technician will use a wand-like device (transducer) over the blood vessels that supply the penis to analyze for blood flow issues. A psychological exam: Your doctor may want to evaluate you for depression or other mental illnesses that may cause erectile dysfunction. It’s true that sexual function declines with age, but recent research shows that many aging men and women engage in sexual activity, and 50 percent of older adults indicate that sex is an important part of their lives. (3) In addition to these findings, researchers indicate that there has been a sharp rise in erectile dysfunction, delayed ejaculation, reduced libido and decreased sexual satisfaction in men under the age of 40. This may be caused by porn-induced erectile dysfunction, a growing problem that’s caused by the ease and accessibility of internet pornography.

The production of testosterone and other hormones naturally decreases with age, and this can disrupt erections. Also, in rare instances, kidney failure and liver disease may throw off the balance of hormones necessary for proper erection and intercourse. In most cases, testosterone replacement therapy can bring levels of the hormone back to normal.
Appropriate treatment options should be applied in a step-wise fashion, balancing invasiveness and risk versus efficacy. If possible, the partner should be involved in the decision-making. The decision depends on the patient preferences and expectations as well as the experience and judgment of the physician. Oral phosphodiesterase type-5 inhibitors are first line therapy.
